Meet Our Team

Jeremy Maddox
PT, DPT, MTC, CSCS

Jeremy Maddox is an owner of Optimal Performance and Physical Therapies, a leading private outpatient physical therapy company with 21 locations across Central West Florida. Under his leadership, the company has grown to become the largest therapist-owned practice in the region, earning recognition as one of Tampa Bay’s Top Workplaces for five consecutive years and being named Best of the Bay in the Physical Therapy category in 2021.

In addition, Jeremy is the founder of Coastal Rehab Academy, LLC. With over 25 years of experience in the field, he holds both a Master’s and Doctorate in Physical Therapy from the University of St. Augustine for Health Sciences. He is certified in Manual Therapy, a Certified Strength and Conditioning Specialist (CSCS), and holds a Bachelor’s degree in Marine Science with a Minor in Biology from Coastal Carolina University.

For the past 18 seasons, Jeremy has served as a Physical Therapy Consultant for the New York Yankees, becoming the first Physical Therapist on staff for the organization. His expertise extends across multiple professional sports, having worked with elite athletes from the MLB, NFL, NBA, NHL, PGA, LPGA, ATP Tour, WWE, and the Olympics.

Jeremy’s treatment philosophy is rooted in providing exceptional, evidence-based care that surpasses patient expectations. His mission is to create an environment of support, healing, and expertise, ensuring that every patient receives the highest level of clinical excellence and personal attention.

Abbey Liebert
PT, DPT, Board-Certified Clinical Specialist in Orthopedic Physical Therapy

Abbey is an orthopedic physical therapist at the Boston University Physical Therapy Center,an on-campus, faculty run physical therapy clinic. She is currently a fellow through the Boston University Manual Therapy Fellowship Program. She completed both her Bachelor of Science in health studies and Doctor of Physical Therapy at Boston University. Abbey also completed an orthopedic residency program at the University of South Florida. She previously served as a physical therapist at an outpatient PT clinic in Tampa, FL where, in addition to her patient care responsibilities, she also mentored new clinicians, provided educational content to coworkers, and helped develop programmatic material to improve patient outcomes.

Abbey has extensive teaching experience working as an adjunct faculty member in Nova Southeastern’s PT program at their Clearwater, FL campus as well as serving as a clinical instructor for PT students. Abbey is dry needling certified and working towards obtaining her FAAOMPT through the APTA. Her clinical interests include pain science, manual therapy, and treating active individuals across the lifespan.

Charlie Domnisch
PT, DPT, Board-Certified Clinical Specialist in Sports Physical Therapy

Charlie Domnisch earned his Doctor of Physical Therapy (DPT) degree from the University at Buffalo and is currently serving as the Assistant Rehabilitation Coordinator for the New York Yankees. In this role, he oversees long-term player rehabilitation programs, coordinates return-to-play protocols, and provides support for in-season injury management.

Before joining the Yankees, Charlie worked as a sports physical therapist at The Ohio State University Wexner Medical Center. There, he consulted for the athletics department and treated professional athletes across multiple sports, including the NBA, NFL, MLB, and professional tennis. Charlie also served as a faculty member and lead mentor for the Sports Physical Therapy Residency and Upper Extremity Fellowship programs.

His clinical interests include the management of overhead athletes, the use of technology to inform clinical decision-making, and the development of throwing programs.

Jack Malafronte
PT, DPT, LAT, ATC, Board-Certified Clinical Specialist in Sports Physical Therapy

Jack is currently an athletic trainer and sports physical therapist for the New England Revolution in Major League Soccer. In this role, Jack provides rehabilitation services for the club and assists with workload monitoring, injury prevention, and return to sport progressions. Jack completed both his Bachelor of Science in athletic training and Doctor of Physical Therapy at Boston University. Jack also received advanced training by completing the sports physical therapy residency program at Texas Health Sports Medicine in Fort Worth, TX. 

He has previously worked with the elite athletes at Texas Christian University, the Premier Lacrosse League, IMG Academy, and Harvard University. He has also privately treated numerous division 1, professional, and Olympic athletes. 

During his time at IMG Academy, Jack served as the site coordinator and lead mentor for the Johns Hopkins Sports Physical Therapy residency program. He remains actively involved in sports specialization through the Specialization Committee within the American Academy of Sports Physical Therapy.

His clinical interests include patients with ACL injury and other post-operative knee diagnoses and hip and groin pain. Jack has previously published peer-reviewed research on knee joint loading after ACL-R and he has lectured on the state and national level on a variety of topics related to sports medicine and rehabilitation.

David Colvin
PT, DPT, MS, ATC, OCS

David is currently the Rehabilitation Coordinator for the New York Yankees. He has been a physical therapist within the organization since 2016. In this role, David provides rehabilitation services for athletes throughout the organization. David completed both his Bachelor of Science in Athletic Training and Doctor of Physical Therapy at the University of Pittsburgh. He also completed a Master of Science in Kinesiology with a concentration in Biomechanics at the University of Tennessee – Knoxville. David gained experience with various sports in the sports medicine departments at both Universities.

Prior to joining the Yankees, David worked as a physical therapist at the UPMC Rooney Sports Complex in Pittsburgh, PA. He also completed a sports medicine internship with the Miami Dolphins during the Summer of 2015.

His clinical interests include injury management of sports-related injuries. David speaks annually at the Baseball Sports Medicine: Game Changing Concepts conference in New York City.
